You must first fill out online the Initial Contact & Reservation Request Form before we can schedule your appointment. This screening process helps me to get a better understanding of your goals, health history, and to create your file.
After I review your information, I will contact you over email or phone to move forward in scheduling your first session.
After scheduling your session, you will need to fill out the Client Intake Form and have it completed for our first session.
